Please call us or email us to book an appointment with Dr Farah.
Alternatively fill in our enquiry form on the Contact page and we will be in touch with you during the business day.
You may also fax us a referral from your general practitioner.
Cancellation Policy
Should you need to cancel your appointment, please notify us at least 24 hours beforehand, to enable us to offer your appointment to patients on the waiting list.
A cancellation fee may be incurred.
Repeat cancellations may require upfront payment for future appointments.
What to bring to your appointment
Please bring any relevant medical information such as your medical history, medication list, investigation results (e.g. Blood test, Xray, ultrasound, CT scans) or any other medical summary.
These documents provide us with valuable information which could help towards an accurate diagnosis / treatment.
Do not forget your referral letter which was given to you by your GP or referring doctor. Dr Farah may not be able to see you if you do not have a referral letter.
Payment Methods
We accept all methods of payment including EFTPOS, credit cards (no AMEX) and cash.
Billing
Dependent on your private health fund, Dr Farah provides a no gap service.
